When I've done the this step, the Bloodbeast spawns, but not where most guides say that it does. It was away from the hill/mountain area to the east a bit. The ghost guy never followed me.

Zone into the normal ToV and there's a big giant guy to talk to at the entrance. Once you have your instance, you can teleport back to it via a Magus (gives a new option when you Hail one with an active instance).
